Member agrees, authorizes and directs Thrive Community Fitness to renew this agreement on a month-to-month dues paying basis and further directs Thrive Community Fitness to continue to access member’s bank account or credit card for monthly dues payments or other fees and charges authorized in this agreement. Member understands that monthly dues may increase subject to at least thirty (30) days notification by the company. Notification may be provided via mail, email and/or in writing at the front desk of the Company’s Fitness Center locations.
Cancellation of Membership or Services: Member agrees to give a minimum of thirty-five (35) days written notice of cancellation by the 25th day of the month to make cancellation effective and to stop Electronic Funds Transfer of owed fees for the next month. With last month’s dues paid, membership is valid through the end of the next month following cancellation notice. Member acknowledges that he/she is liable and agrees to pay all fees and charges paid or accruing during the thirty-one (31) days prior to the next billing date. Written notice must be physically delivered to the Thrive Community Fitness facility where your membership was purchased. Oral cancellation is not valid. There are no refunds given on services, last month’s dues, or annual membership fee’s.
